I only like to play safe, so I brought my own condoms and put them on the bed.

Adressing myself mainly to those living here or spending a lot of time here, but could somebody give me some good addresses where you can get dental implants?

I know if I look on the net I will probably find zillions, but I would like to get some first hand information on service, price, quality etc..

Altough healthcare is top notch in my country dental implants are not covered by the health care system, and are quite expensive even if you have a dental insurance plan, they are only reimbursed till a certain amount.

On my last trip "ianoxon" from the Pattaya forum suggested that a Cholera vaccine offers some protection from food poisoning, so this post follows on from that suggestion and I have done is a bit of AI research and considered the benefits for me for future trips.

So here are some details that I have found out (cut and pasted from a long-winded AI series of questions and clarifications):

The Cholera vaccine protects against Cholera and enterotoxigenic E. coli (ETEC)   that produce heat-labile toxin — these can cause a type of traveller’s diarrhoea.

There is no/ZERO protection from other forms of food poisoning such as  

• • Campylobacter
  • Non-ETEC E. coli
  • Salmonella
  • Shigella
  • Norovirus
  • Staph toxins
  • Parasites (Giardia, etc.)

Having said that in “my view” the use of untreated water to clean utensils and prepare food combined with our hobby puts us more exposed to this type of food poisoning/infection than perhaps a sexually inactive tourist of say Europe (with treated water and higher food hygiene standards).

Apparently ETEC is one of the most common causes of traveller’s diarrhoea in Southeast Asia and we are maybe at an increased risk. So the vaccine itself can offer partial, short-term protection against ETEC-caused diarrhoea. This short term appears to be about 3 months.

For ETEC traveller’s diarrhoea (the partial protection):

• Peak protection: in the first 1–3 months after completing the course
• Meaningful protection fades after ~3 months

Virtually gone by 6 months (but still effective for Cholera for a total of 2 years)

The vaccine is taken orally in 2 doses

Ideal schedule:

• Dose 1: about 3–4 weeks before travel
• Dose 2: about 1–2 weeks before travel
• You are protected starting 1 week after dose 2

Booster

• One dose, at least 1 week before travel (Only valid if your last course was within 2 years)

So, this to me seems a valid and useful option for annual travellers and not so useful for residents (who perhaps have hardened up and have a bit more tolerance to local water/food than us blow-ins).

Anyway, my next trip is planned for early next year and I will be dosed up with Dukarol (the Cholera Vaccine) and hopefully avoid getting sick.

Is there a good alternative to Vigour 800?
One of the problems of Vigour is loosing the sensitivity.

Gilead, a California based pharmaceutical company, announced on Thursday (20 June 2024) that it was ending the phase 3, blinded clinical trial of its new, twice-a-year shot for pre-exposure prophylaxis (PrEP) against HIV, lenacapavir, because it is too effective.

In this sort of clinical trial, some participants are randomly assigned to the new intervention that's under investigation (i.e. lenacapavir) and some are randomly assigned to the current standard of care (i.e. Truvada or Descovy, PrEP pills that must be taken every day). Why? Because it would be unethical to use placebo when we know that once-daily PrEP pills do prevent HIV.

The trial is also "blinded" to prevent investigator bias from influencing the results – to the maximum extent practicable, the investigators carrying out the research do not know, while the trial is being carried out, which participants are getting which treatment, nor do they know how effective those treatments are.

But an Independent Data Monitoring Committee (IDMC) has access to all of the data. The IDMC can see, while the trial is ongoing, which participants have gotten which interventions, and they can see how effective those interventions are.

The IDMC saw that none of the 2,134 women who received the twice-yearly injection of lenacapavir developed HIV. 0 cases of HIV in any of the participants assigned to lenacapavir.

There were 16 cases of HIV in the 1,068 women assigned to Truvada. There were 39 cases of HIV in the 2,136 women assigned to Descovy.

At that point, the IDMC has an ethical duty to recommend the blinded trial be stopped, and all of the participants assigned to Truvada and Descovy be offered lenacapavir. Why? Because they know lenacapavir works, it is superior to either Truvada or Descovy, and once that fact has been established, it would be unethical to deny participants the option to receive the more effective PrEP option of lenacapavir.

Lenacapavir is not the first injectable PrEP agent. FDA approved "apretude" in 2021; it must be injected every other month. The game changing nature of lenacapavir is that it only requires two injections per year in order to achieve 100% efficacy.

So off I trotted to MedConsult which is located on soi 49/9, right in the heart of a very Japanese area. 

The clinic is on the third floor of the Racquet Club, which is located on the 9th alley off soi 49. A good landmark is the Sumitvej Hospital, located on soi 49 just a few minutes before the alley 9.

I'd recommend MedConsult as the main doctor there is a Kiwi lady ('Doctor Donna') and it is run along farang lines. I'd say that 99% of the patients are farangs and it is a busy practise focused on work medicals, vaccinations, aesthetics, prescriptions and STD check-ups! 

It is low cost and so if ever you are in need of any of these services, I would recommend it, as it is highly efficient in my experience. (The blood tests etc. are a LOT cheaper than Pulse Clinic!).

It is also open 7 days a week and has a big aesthetics section. I got the feeling it would be 100% fine to walk in there with a LB, no problems.

I was at Bangkok Hospital earlier this week and the doc was a very chatty fellow, with good English. He asked if I'd had the 'flu jab yet and I advised I had it in SA in March. It seems that they are expecting a much higher prevalence of 'flu this year in the LOS and have imported extra supplies. In the northern hemisphere the 'flu season is the winter, but in the LOS it coincides with the rainy season, which will start in the next few weeks. (He advised always get the 'flu jab between Jan-May each year, as it takes up to 4 weeks to become effective).

The conversation then moved on to Covid-19 and he advised there had been a spike up to 1,000 new cases a day during Songkran and so they are also expecting an increase in the rainy season. He advised if you are 60+, haven't had a jab, or haven't been infected in 6+ months, then you should also get a booster.

The Experience

They are also running the same scheme at CentralWorld, but the times are 1500-2100. I decided that MBK would be a better option, as hopefully there would be less people during the day.

MBK is very easy to get to from the 'Combat Zone'. Take the BTS to Siam. Change trains and take the train to National Stadium. Then follow the signs to MBK and there is a direct walkway which takes you onto the 2nd Floor. Take a lift or escalator to the 6th floor, which is spookily deserted. Walk to the far end and you will see the temporary vax centre.

Present you passport. If you are a local or a resident visa holder its free. I was the only farang; but there were quite a few Asian tourists and they have to pay a nominal fee (THB 490? I'm not certain).

You fill out a form, in English (that was impressive) and then walk in. You take a seat and then called to another room and get jabbed. Then on the way out you have to hand in the form and are directed to a "recovery room" for 30 minutes. Don't skip this, as I was tempted to. This is where they print and give you the certificate.

I visited today and it was a 50 minutes process - 20 minutes for the actual execution and 30 minutes for the 'recovery'.

I deliberately chose 10.30 to avoid crowds, and it was not busy.

The vax is intended as a booster and is the Moderna version with the Omicron strain. All of my previous 3 Pfizers were for the previous strain, hence why I went.

I'd recommend doing this if you believe in the vaccination route and the process was well organised and ran smoothly. They are geared up for large volumes, so it was a pleasure today.

They are running this daily until Friday 12 May, 0900-1500 and then for the 2 weekends after the election. If you have a full time job, CentralWorld is doing the same days, but 1500-2100, so that may fit better for you.
